The report is modeled on a real water system audit and flow test, with a compact new-well section that stays blank on routine service. A tech doing a simple inspection fills only what applies — empty fields never print on the emailed report.

  • Job type first. Inspection / flow test, repair, install / upgrade, or new well — the rest of the form follows the work.
  • Well details. Drilled, bored, driven or dug; casing size, estimated depth, pump set depth and static water level.

Static level, pumping (drawdown) level, flow rate in GPM, pumping duration, max pump pressure and drop pipe size — with a free-text field for staged readings (levels / GPM / psi / amps) as the test runs.

Pump type (submersible, jet, booster, constant-pressure), HP, volts, motor max amps vs measured amps at load, winding resistance and insulation-to-ground in ohms, and the control box date.
